Neighborhood Health Association (NHA) is committed to serving the needs of its patients and provides a Sliding Fee Schedule to assist both uninsured and under-insured patients.
MINIMUM FEE FOR SERVICE
If you are uninsured or underinsured patient at NHA, you will be charged a discounted rate based on the current Federal Guidelines established to assist you.
Please remember that if you need assistance to bring most recent 2 payroll stubs to your appointment and identification with you.
Payment for services is expected prior to seeing the doctor at the time of service; the minimum co-pay is due each time.

Please talk to our appointment desk or a social worker. If you need to make special payment arrangements, please also ask about our sliding fee scale form that may be available which can reduce our already affordable services.
MOST INSURANCE TYPES ACCEPTED
We accept most types of commercial insurance, including Medicare, Medicaid, and others. Please check to ensure that your insurance is among those we cover.
