DAISY SMITH PEDIATRICS

At Neighborhood Health Association our goal is to provide the highest quality healthcare for your child. We have 2 pediatric centers, several school based programs, and a high school health clinic to see your child for their medical care.

EXPECTING MOMS

If you are pregnant and seeking doctors for your newborn contact us now to acquaint yourself with our providers and medical care team.

WELL-CHILD CHECKUPS

Children who see their doctor regularly for well-child checkups and receive their immunization shots on schedule will positively effect their growth and development.

SERVICES

NHA Peds and Daisy Smith Pediatrics offer full comprehensive medical care for your children.

• Child Well Check Examinations
• Vaccinations (measles, mumps, chicken pox, and other infections treatment)
• Physical examinations (sports, school, and employment)
• Treatment of childhood chronic diseases (asthma, allergies, childhood diabetes)
• Treatments for Accidents and Injuries
• Vision and Hearing screenings
Women, Infant and Children (WIC) program

EVERY DOCTOR’S APPOINTMENT

Parents should bring for their child/children the following information and documents to each doctor’s appointment:

• Child's shot records
• All Prescription medication bottles
• Please make sure to tell your nurse or doctor if you need a prescription re-fill or any problems with drug interaction
• A list of all over the counter medication taken
• A list of any allergies
• Questions for the doctor to answer while at your visit
• Health Insurance Card or Cash to pay for your visit
• Any paperwork that the doctor needs to complete on your behalf
• All requests for paperwork will take 7-14 working days to complete

YOUR CHILD/CHILDREN FIRST APPOINTMENT

• Arrive EARLY to complete new patient paperwork

• Bring $20.00 for your minimum payment if your child does not have insurance

• Bring your child’s health insurance card

• To qualify for the Sliding Fee Scale benefit bring two recent paychecks.

• If you are not working, visit Lucas County Jobs and Family Services to receive documentation showing you are not receiving any benefits.

NHA accepts the following financial arrangements for medical services including Medicare, Medicaid, Commercial Insurance plans and a Sliding Fee Scale for Self Pay patients (which incorporate a patient's family size and income for eligibility).
